" SEC. 341. (a) The Commissioner shall c a r r y out a program for making g r a n t s to States under State plans approved under section 307 to provide in-home services to frail older individuals, including in-home supportive services for older individuals who a r e victims of Alzheimer's disease and related disorders w i t h neurological and organic b r a i n dysfunction, and to the families of such victims. "(b) In carrying out the provisions of t h i s part, each a r e a agency shall coordinate with other community agencies and voluntary organizations providing counseling and t r a i n i n g for family caregivers and support service personnel in m a n a g e m e n t of care, functional and needs assessment services, assistance with locating, a r r a n g i n g for, and coordinating services, case m a n a g e m e n t, and counseling prior to admission to n u r s i n g h o m e to prevent p r e m a t u r e institutionalization.

"DEFINITIONS

> " SEC. 342. For purposes of t h i s part— 42 USC 3030i. "(1) the term 'in-home services' includes— "(A) h o m e m a k e r and h o m e health aides; • "(B) visiting and telephone reassurance; vi t "(C) chore m a i n t e n a n c e; "(D) in-home respite c a r e for families, and a d u l t day care as a respite service for families; and

"(E) m i n o r modification of homes that is necessary to facilitate the ability of older individuals to r e m a i n a t h o m e and that is not available under other programs, except that 2i not more than $150 per client may be expended under this '•'•'part for such modification; and "(2) the term 'frail' means h a v i n g a physical or m e n t a l disability, including having Alzheimer's disease or a related disorder w i t h neurological or organic b r a i n dysfunction, that restricts the ability of a n individual to perform n o r m a l daily t a s k s or which t h r e a t e n s the capacity of a n individual to live independently. " SEC. 343. The State agency shall develop eligibility criteria for providing in-home services to frail older individuals which shall t a k e into account—

"(Dage; "(2) greatest economic need; "(3) noneconomic factors contributing to the frail condition; and "(4) noneconomic and nonhealth factors contributing to the need for such services.
