SEC. 9. The work or vacation permit required by this Act shall be issued only by school issued only by the director of the department of school attendance att enda nce, etc ., officer . I t shall certify that all the requirements for issuing a work or vacation permit under the provisions of this Act have been fulfilled and shall be signed by the person issuing it . It shall state the name and address of the employer for whom and the nature of the specific occupation in which the work permit authorizes the minor to be employed, and no permit shall be valid except for the employer so named and for the occupation so designated . It shall bear a num- ber, shall show the date of its issue, and shall be signed by the minor for whom it is issued in the presence of the person issuing it, and shall be mailed to the employer by the issuing officer, and in Record to be kept. no case given to the minor . A record giving in full for each appli- cant the facts with reference to his sex, color, date, and place of birth, name and address of parent, guardian, or custodian, name and address of employer, and nature of the specific occupation in which the minor is to be employed, grade and school last attended, evi dence of age, and date of i ssuance o r date of refusal of certif icate, with reason, shall be kept in the department of school attendance and work permits, together with the physician's certificate of physi- cal fitness, the school record, and the employer's statement of inten-
