Dear Mr. Halaby:

On October lh, 1963, a New York Airways hellcopter, Vertol lOT, N 6673B, crashed at New York Internatlonal Alrport as the result of a fatlgue fallure 1n the gulll shaft, P/N lOTD206T—l, S/N anloe, which connects the mlx box to the aft transmlss1on.

Slnce many representatlves of the Federal Av1ation Agency are worklng w1th us 1ntensely, 1t 15 unnecessary to 1nclude 1n thlS letter numerous detalls relatlve to the facts disclosed by'the 1nvest1gatlon to date. Should the Admlnlstrator permlt resumptlon of passenger Operatlon 1n New York Airways helic0pters, 1t 15 recommended that the follow1ng actlon be accompllshed as outllned 1n paragraphs A, B, and C below, untll the cause of the abnormal wear 1n the spllnes of the qulll shaft 15 determlned and approprlate changes are made on the hellcopters or 1n pertlnent procedures affecting the safety of operatlon.

A Overhaul of aft transmlsslon and mlx box, 1nclud1ng 1nstallat10n of a new qulll shaft P/N lOTD 2067-(?), prlor to resumptlon of operatlons

B. Reduc on of overhaul perlod for above assemblles to 200 hours.

C Overhaul of above assemblles prlor to next flight 1n case of sudden englne stoppage or other abnormal loadlng of aft transm1551on and/or mlx box.
