(a) A statement signed by the prospective employer or by some one From employer . duly authorized on his behalf, stating that he expects to give such Details . minor present employment, setting forth the specific nature of the occupation in which he intends to employ such minor, and the number of hours per day and of days per week which said minor shall be employed, and agreeing to send the notice of the commencement of employment, and to return the permit according to the provisions of this Act. (b) Evidence of age as provided in section 11 of this Act, showing Evidence of age . that the minor is at least fourteen years of age . (c) A certif icate of physical fitness, if such m inor is u nder sixt een tis Phy sical fitness cer- years of age ; otherwise no such certificate of physical fitness shall be re quired . It shall show the Details of certificate . height and weight of the minor and shall state that the said minor has been thoroughly examined by the said physician at the time of his application for a permit, has attained the normal development of a minor of his age and is in sound health, and is physically qu alifie d for the em ployme nt spe cified in the state ment s ubmitt ed in ac cordan ce wit h the requir ements of th is Act . It shall Contents . certify that the said minor is able to read and write correctly sentences in the English language, has satisfactorily completed the eighth grade of the elementary school course prescribed for the ublic schools in the District of Columbia, or has regularly received - in a private or parochial school instruction deemed equivalent by the Board of Educati on to that p rescri bed fo r the complet ion of the e ighth grade in the public schools. Such school record shall give also the full name, date of birth, grade last completed, and residence of the minor as shown on the records of the school . SEC . 11 . The evidence of age required by this Act shall consist of q ired . of ag e re one of the following proofs of age, which shall be required in the order herein designated (a) A birth certificate or attested transcript issued by a registrar Birth certificate. of vital statistics or other officer charged with the duty of recording births. (b ) A ba ptisma l reco rd or duly c ertifi ed tran script there of sho w- Baptismal record, ing the date of birth and place of baptism of the minor . (c) A bona fide contemporary record of the date and place of the Family record, etc . child's birth kept in the Bible in which the records of the births in the fam ily of the c hild a re pre served , or ot her do cument ary ev idence satisfactory to the director of the department of school attendance and work permits, such as a passport showing the age of the child,
