(c) The amendments made by subsections (a) and (b) shall be applicable with respect to monthly insurance benefits under title II of the Social Security Act beginning with the second month following the month in which this Act is enacted but only on the basis of an application filed in or after the month in which this Act is enacted.

DISCLOSURE, UNDER CERTAIN CIRCUMSTANCES, TO COURTS A N D INTERESTED WELFARE AGENCIES OF WHEREABOUTS OF INDIVIDUALS

SEC. 340. Section 1106 of the Social Security Act is amended by ^J Jta^ 5 5 ^ ^9 adding at the end thereof the following new subsection: "(c)(1) Upon request (filed in accordance with paragraph (2) of this subsection) of any State or local agency participating in administration of the State plan approved under title I, IV, X, X IV, X VI, or X IX, or participating in the administration of any other State or 42 USC 301 et local public assistance program, for the most recent address of any ^*'^* individual included in the files of the Department of Health, Education, and Welfare maintained pursuant to section 205, the Secretary ^2^*|l* ^Q^^' shall furnish such address, or the address of the most recent employer, or both, if such agency certifies that— " (A) an order has been issued by a court of competent jurisdiction against such individual for the support and maintenance of his child or children who are under the age of 16 in destitute or necessitous circumstances, '^(B) such child or children are applicants for or recipients of assistance available under such a plan or program, " (C) such agency has attempted without success to secure such information from all other sources reasonably available to it, and " (D) such information is requested (for its own use, or on the request and for the use of the court which issued the order) for the purpose of obtaining such support and maintenance. "(2) A request under paragraph (l) shall be filed in such manner and form as the Secretary may prescribe, and shall be accompanied by a certified copy of the order referred to in paragraph (1)(A). "(3) The penalties provided in the second sentence of subsection (a) shall apply with respect to use of information provided under paragraph (1) of this subsection except for the purpose authorized by subparagraph (D) thereof. "(4) The Secretary, in such cases and to such extent as he may prescribe in accordance with regulations, may require payment for the cost of information provided under paragraph (1); and the provisions of the second sentence of subsection (D) shall apply also with respect to payment under this paragraph." SEC. 341. (a) Clause (B) of section 1402(e)(2) of the Internal Revenue Code of 1954 (relating to time for filing waiver certificate by 7g^stft^\*o76^' ministers, members of religious orders, and Christian Science practi- 26 USC 1402. tioners) is amended by striking out "his second taxable year ending after 1962" and inserting in lieu thereof "his second taxable year ending after 1963". (b) Section 1402(e)(3) of such Code (relating to effective date of l^H^^ ^ "; certificate) is amended by adding at the end thereof the following ^^ new subparagraph:
